Care Club nursing service exists to provide effective service to our members, by using the nursing process, to achieve maximum health. It is our objective to maintain or restore an individual's optimum state of physical, mental, emotional, spiritual and social well-being, in order to maintain his/her involvement in his/her community environment.
The objectives of the nursing service will be achieved by the Program Nurse through the following strategies and responsibilities:
- Participate in the multidisciplinary team to achieve health care goals formulated by members of the health care team and the individual member/family.
- Assure implementation of the nursing component of the plan of care for each member based on the identified model of nursing practice.
- Acknowledge and protect the rights of individual members as recipients of health care services.
Care Club Adult Day Care Center specializes in care for adults who need support. Club members of the Day program may experience one or more of the following:
- Physical impairments due to stroke, and other neurologic diseases
- Early Stages of Alzheimer's Disease
- Parkinson's Disease
- Developmental disabilities
- Medication Management
- Diabetes
